Apple has today released macOS Tahoe 26.4.1, bringing bug fixes, including for an issue that prevented Macs with the M5 series of chips from connecting to 802.1X networks when content filtering extensions were in use.
As macOS Tahoe 26.4.1 was not distributed to external testers prior to its release, we do not have any information on whether it contains any accessibility fixes for blind, deaf blind, or low vision users at the time of writing. However, as this update appears to have been released to address a small number of specific issues, we do not expect it to contain any such fixes.

How to Update
To install macOS Tahoe 26.4.1, choose System Settings from the Apple menu, select General in the table, click Software Update in the scroll area, and click the Update Now button to begin the update process. If other updates are available, you can click the Info button to see details about them and select specific updates to install.

Dialogs
Has anyone else had trouble with dialog boxes? As in modal ones. Some of this happens with apps I've vibe coded so I'm less sure about how the dialog is made, but I've also found similar things elsewhere, eg in file save dialogs.
Firstly, if grouping is disabled you must interact with the dialog, which was not the case prior to tahoe.
Then if the dialog has a text box, possibly if it's the first thing in the dialog, but not necessarily, then VoiceOver doesn't speak anything in it. So you can't know where the cursor is, nor what text you are entering. It is possible to type, select all and replace etc, but you only know what the text is if you vo left+right or something to hear it. So if there's a typo you basically have to start again.
It's very annoying but I don't have a concrete example showing it outside my own apps - I'm sure I've seen it in Outlook and other places but I've not yet spent the time investigating.
I've also noticed that file dialogs are a bit random - sometimes I can navigate to the file list and just use up/down arrows and typing to find things. Other times only vo up+down works and I can't jump to a filename by typing. I think restarting VoiceOver fixes it if this happens.
I keep meaning to spend more time trying to reproduce this in a way I can send to apple, although I guess they don't care about tahoe any more. I would guess it's still happening in golden gate given that these sorts of things stick around for years on end.
